Enrollment Instructions

 

  1. Please fill out the following form. Once you hit submit, you will be redirected to a SQUAREUP payment portal.
  2. On the payment portal,  click the “+ sign”  if you are enrolling multiple children.
  3. You will enter your phone number and address on the payment portal (under the shipping address field)
  4. Once you complete payment, we will email you with an emergency contact/medical form as well as a waiver.
  5. Only after we receive payment AND waiver is your spot reserved!
